Karl Sanders, guitarist and vocalist of Nile, says he underwent multilevel spinal fusion surgery on his neck on October 1 and is now recovering. He shared the update in a social media post, which also included a photo of his surgical scar, according to New Noise Magazine, which reported the statement on October 26, 2026.

Sanders says he intends to rejoin Nile's European tour, and that he will give up one longtime habit of metal performance: headbanging.

What Sanders says led to the operation

In his statement, Sanders attributes the problems to disc degeneration, decades of headbanging, a martial arts injury 11 years ago and a long stretch of stress. He says he had been living with pain from pressure on his spinal cord and nerves, but that constant numbness in his right hand this year convinced him he could not put surgery off.

Scans, he says, showed severe narrowing around the spinal cord and signs of injury to the cord itself. He says his doctors told him that without surgery he faced further permanent damage, including a risk of paralysis.

Sanders describes the procedure as removing the damaged discs and the bone pressing on his spinal cord. Donor bone was placed between several vertebrae and secured with a metal plate and screws. "So there's officially a little more Metal in me now. \m/" he wrote.

Recovery and the road ahead

Sanders says the surgery went well, the neck and shoulder pain is easing and his throat is healing. He says he is walking two to three miles a day, doing occupational therapy for his right hand three times a week and "playing LOTS of guitar."

"Eight weeks to the day after surgery, I'll be back on the road for our European tour. I'm looking forward to getting back out there."

He also addressed the financial side of touring. "Making music is what I love, but it's also how I make my living, and metal doesn't come with paid medical leave," he wrote. On the change to his stage show, he said: "I won't be doing any more onstage headbanging, but that's OK. I have done more than my share of headbanging during my career, and I am thankful that I can still actively tour and Make Metal for the fans."

Underworld Europe dates

The itinerary published by New Noise lists 19 shows. It starts November 29 at Fuzz Club in Athens, runs through Poland, Slovenia, Romania, Hungary, the Czech Republic, Italy, Belgium, the Netherlands and Germany, and ends December 20 at Kwadrat in Krakow. The only UK date is December 12 at The Dome in London. Other listed stops include Metropool in Enschede, Turock in Essen and Progresja in Warsaw.

The lineup named in the report is Sanders on guitars and vocals, George Kollias on drums, Brian Kingsland and Zach Jeter on guitars and vocals, and Adam Roethlisberger on bass and vocals.

The report does not say whether any dates have changed because of Sanders' recovery.
